SUMMARY This paper presents a factual and critical account of our present knowledge of bilirubin metabolism in human disease. The biochemistry of the bile pigments is reviewed as it is related to various clinical states ; aspects of hepatic diseases, hepatotoxicity, hæmolytic disorders, abnormal pregnancy and fætal and neonatal life are discussed. The clinical biochemistry of familial disorders of bilirubin metabolism is also outlined.
