Summary A case of generalized scleroderma with pulmonary manifestations is described. Completely normal findings on detailed studies of respiratory function three and a half months before the patient's death allow the pulmonary lesions to be described as acute diffuse interstitial pulmonary fibrosis. The histological appearances in the lungs are believed to represent the earliest manifestations of "scleroderma lung" yet described.
