A 42‐year‐old woman developed a superficial basal cell epithelioma in a smallpox vaccination scar. After reviewing similar reports in the English literature, I believe any unusual change occurring in a vaccination scar, either simultaneously or years later, should be viewed as a possible malignancy and treated accordingly.
