Mini‐Abstract We report a case of a 65‐year‐old woman with chronic primary mitral regurgitation diagnosed 10 years prior to our evaluation. We performed a 3D transesophageal echocardiography that showed an isolated cleft within the P2 segment of the posterior leaflet associated with severe mitral regurgitation. This is a rare congenital cause of mitral regurgitation, and it may occur in isolation or with associated congenital heart defects. The slitlike appearance of the leaflet can be difficult to assess by 2 DTTE or even TEE . Real time 3 DTEE provides superior diagnostic accuracy in the identification of scallop‐specific anatomy.
