Mini‐Abstract We present a patient with acute left ventricular free wall rupture and pseudoaneurysm formation after myocardial infarction. We review the accuracy and incremental utility of real time handheld 3‐dimensional transthoracic echocardiography for the assessment of left ventricular rupture location, orifice geometry, and complex intracardiac flow.
