Mini‐Abstract A 61‐year‐old man with no medical history presented with necrosis of the fingers and toes in the setting of newly diagnosed multiple myeloma with profound eosinophilia. A crescendo–decrescendo murmur consistent with aortic stenosis was noted on exam. Transthoracic and transesophageal echocardiography revealed large mass on aortic valve resulting in severe stenosis. Imaging studies and extensive infectious workup were unremarkable for other underlying malignancy, evidence of embolism, infections, or connective tissue disease. Following dexamethasone treatment to achieve a normal eosinophil count, the patient underwent bioprosthetic aortic valve replacement. Pathologic examination was consistent with nonbacterial thrombotic endocarditis.
