Mini‐Abstract Ostium secundum atrial septal defect is a common congenital anomaly. Treatment is indicated in patients with hemodynamically significant shunt and right ventricular failure. The Amplatzer septal occluder is one of the most commonly used devices for percutaneous closure of this type of defects. Embolization of the device to the systemic circulation is a rare but serious complication. We present a case of fatal embolization and entrapment of an Amplatzer atrial septal occluder device in the mitral valve apparatus in an elderly woman 4 days after implantation.
