Mini‐Abstract We present the case of a 42‐year‐old man with incidentally noted dilatation of the coronary sinus who underwent bilateral upper limb agitated saline injection to confirm a diagnosis of persistent left and absent right superior vena cava. The case highlights the ability of selective bilateral upper limb agitated saline injection during transthoracic echocardiography to conclusively characterize venous return patterns. Anatomical confirmation of the diagnosis was subsequently made by thoracic computed tomography.
