123 Caucasian subjects (91M, 32F, mean (SD) age 53.1 (9.2)yrs, BMI 29.1 (4.6)kg m"2) with newly diagnosed type 2 diabetes and no evidence of diabetic retinopathy (DR) were studied for 5yrs (ethical approval obtained). Risk factors considered included gender, age, treatment and family history of type 2 diabetes; Oyr, 5yr and change in: BMI, HbA,, lipids, blood pressure, GFR, ERPF, creatinine, urea, insulin sensitivity, !}-cell function, smoking status; fasting, absolute and incremental areas under the curve (AUC, MUC) for meal tolerance test (MTT) plasma glucose, C-peptide, insulin and insulin secretion rate data. Genderxvariable interactions were also included. The 5yr incidence of DR was 23/123 (18.7%). 21/23 subjects had background DR, 2123 had maculopathy. After 5yrs, 45/123 (36.6%) subjects were treated by diet (4/45 (8.9%) developed DR), 74/123 (60.2%) by oral hypoglycaemic agents (17174 (23%) developed DR) and 4/123 (3.2%) by insulin (214 (50%) developed DR). At Oyrs, the 5yr-DR group had significantly lower Jl-cell function, higher glucose and lower C-peptide, insulin and insulin secretion rate MIT AUCs and MUCs than the no-DR group (p
