The balance between specificity and sensitivity in triage cytology must take into account that the first is the crucial point for the process of cervical cancer screening using HPV primary screening test. As the use of HPV as the primary screening test has completely changed the role of the Pap test in the screening algorithm, should “The Bethesda System for Reporting Cervical Cytology” be reviewed? In particular, is the use of the ASC‐US category still truly necessary?
