Spindle epithelial tumor with thymus‐like elements is a quite rare and enigmatic tumor of uncertain histogenesis occurring within the thyroid. The authors present an illustrative case in a young female highlighting the cytomorphology, immunohistochemistry and molecular features, and discuss the diagnostic difficulties and therapeutic uncertainties encountered.
