Pilomatricoma is a relatively rare, benign skin appendageal neoplasm with predilection for head and neck region, young age, and female sex. Cytological diagnosis is often challenging owing to the lack of awareness and failure to identify the characteristic cytomorphological features, often leading to misdiagnoses. Development of multiple pilomatricomas is very rare, with only a few cases reported in the literature. We report a case of a young woman presenting with multiple pilomatricomas, which could be accurately diagnosed on fine needle aspiration cytology.
