The cytological diagnosis of a monomorphic synovial sarcoma can be challenging, especially when it presents in a rare intramuscular location. As synovial sarcoma is characterized by the specific balanced chromosomal translocation t(X;18)(p11;q11), an integrated cytological and molecular approach is considered mandatory.
