Accessible summary A user‐friendly, client‐centred tool was developed to enable service users to set goals about activities at home and in the community. Using this tool benefitted service users by greater involvement in goal setting; more independence, choice and control; higher motivation. We will continue to improve this tool to meet the needs identified by service users of all ages.
