Summary Simple intratesticular cysts are a rare condition. High resolution sonography permits unequivocal sonographic differentiation between intratesticular cysts and neoplastic lesions with a cystic appearance, so that conservative treatment seems justified. Seventeen patients with sonographically diagnosed intraparenchymal testicular cysts were followed up for an average of 29 months. No increase in size or change in echo pattern was found and surgical intervention was therefore not indicated.
