On the cover: Chronic lung allograft dysfunction (CLAD), a term that denotes the primary pathway by which lung allografts are lost, remains poorly understood as it encompasses multiple clinical and morphological subtypes. In this issue, Verleden et al ( page 2644) review, from a tissue‐centric perspective, recent discoveries elucidating the pathophysiological mechanisms of bronchial, alveolar, and vascular injury that, alone or more frequently in combination, contribute to lung allograft demise. Emerging from this cogent minireview is a perspective that the primary subtypes of CLAD, bronchiolitis obliterans syndrome, restrictive allograft syndrome, and chronic antibody‐mediated rejection, are likely inter‐related and overlapping, reflecting the push and pull of tissue injury and remodeling in the various lung compartments.
