On the cover: Although the management of acute cellular allograft rejection has markedly improved over the past two decades, alloantibody formation remains a formidable long‐term dilemma for all transplanted organs. This month, Mark Haas provides an exceptional overview ( page 2849) of the progression of de novo alloantibody formation in kidney transplantation, which adds a useful guide for the clinical management of antibody‐mediated disease as well as a thoughtful framework for much needed discovery work in this area. In a related article, McCaughan et al ( page 2924) report on the identification of epitope mismatches that increase the risk of de novo donor‐specific HLA antibody development in cardiothoracic transplantation. Also, don't miss the annual “What's hot, what's new” article summarizing the top findings from the 2018 American Transplant Congress ( page 2857). Cover design by Lauren Halligan, Duke University Department of Surgery .
