On the cover this month: Organ failure is thankfully uncommon in children, but when it does occur, organ transplantation is typically the best therapeutic option. Since pediatric transplantation is a relatively low‐volume specialty, it has historically been challenging to develop adequately powered clinical research. Fortunately, the NIH has provided a superb collaborative mechanism (the Clinical Trials in Organ Transplantation in Children; CTOTC) to support transplant research in children, and this month we feature a series of reports from the cardiac CTOTC‐04 trial investigating the consequences of preformed and de novo alloantibodies in pediatric heart recipients (see Zuckerman et al, page 2135; Webber et al, page 2148, and Dipchand et al, page 2163). We present these articles in the context of many others addressing pediatric transplantation in other organ systems as well as a complementary series of studies addressing the challenges of alloantibodies more broadly.
