On the cover this month: With any scarce resource, access becomes a dominant feature of its use. This is certainly the case for deceased donor organs, and the transplant community has worked continuously to examine and refine the policies governing access to transplantation. In this issue, Stewart et al ( page 1924) provide new methodology to measure unintentional disparity in listed patients’ access to transplantation, demonstrating that disparity related to a patient's allosensitization (cPRA) has fallen significantly since the implementation of the Kidney Allocation System (KAS). This is presented along with an independent analysis by Zhang et al ( page 1936) examining the impact of the KAS on access to the waiting list, work by Martins et al ( page 1947) examining disparities related to implementation of the A2 to B program, and a new educational aid (Patzer et al, page 1954) designed to inform a patient's decision to consider transplantation.
