On the cover this month: A major feature in organ transplantation is a period of relative organ ischemia followed by a reperfusion event. This evokes variable degrees of organ injury proportionate to the length of the ischemic episode that, when severe, canprevent organ function and lead to graft loss. This month, Kageyama et al ( page 1755) highlight the functional significance of the Notch1 signaling pathway in hepatoprotection against the ischemia reperfusion stress response and the ensuing innate immune response in both mouse and human liver transplantation. They also offer a suggestion on a therapeutic strategy using serelaxin, a recombinant version of the molecule human relaxin‐2. James Markmann provides a thoughtful perspective in his editorial (page 1587). Also, we feature a meeting report from an AST‐ASHI expert workgroup that provides recommendations on the definition and utilization of HLA diagnostic testing, as well as a framework for clinical assessment of risk for a memory or a primary alloimmune response.
