On the cover this month: In all things, there is a balance between risk and benefit, and humans are by nature risk‐averse, particularly in transplantation. Although we frequently understand the risk of specific actions, the opportunity costs and risks of inaction are often less apparent. In this month's issue of AJT , Bowring et al ( page 617) look at the risks of turning down kidneys labeled as increased risk for disease transmission and show that the acceptance of such kidneys actually confers a survival advantage compared to turning the kidneys down and waiting for another offer. Daniel Kaul provides a nice editorial on this topic ( page 535), which inspired this month's cover art. Also look for an informative Meeting Report from Ettenger et al ( page 564) detailing the proceedings and insights from a recent FDA public meeting on drug development and medication adherence.
