On the cover this month: Despite considerable attention, lung transplant outcomes have lagged behind those of most other organ types; the cause has been multifactorial, including pretransplant, perioperative, and long‐term challenges. This month, we highlight numerous articles demonstrating advances in this field. These range from new insights in managing critically candidates for transplantation on ECMO to significant advances in the diagnosis of rejection and the biology of graft acceptance. New thoughts regarding the evolving metrics governing the practice are also advanced. As always, these articles are accompanied by thoughtful editorials, which place these advances in context.
