On the cover this month: Costimulation blockade has long been most effective in experimental models when CD28‐B7 pathway blockade has been combined with CD40‐CD154 pathway blockade. Although CD28‐B7 blockade can be obtained clinically with belatacept, the CD40‐CD154 pathway has not navigated clinical trials. This month, Kim et al ( page 1182) report on an Fc‐silent CD154‐specific domain antibody (dAb) that safely and effectively promotes long‐term kidney transplant survival in nonhuman primates without the risk of thromboembolism when combined with a clinically relevant conventional immunosuppression regimen. On our cover is a 3D rendering of a CD154‐specific dAb (left) binding to a CD154 trimer. This may open the door to clinical translation of a new agent to be paired with belatacept in the clinic.
