Summary Portomesenteric venous thrombosis is a rare but potentially life‐threatening condition. The presenting symptoms of chronic portomesenteric venous thrombosis are often non‐specific but may present with variceal bleeding. We present the first reported case of chronic portomesenteric venous thrombosis causing a high flow arteriovenous malformation that resulted in extensive gastrointestinal bleeding.
