Upon routine examination of the donated cadaver specimen we observed a donor body with severely shortened left lower limb. While proximal femoral focal deficiency has been reported in the literature extreme femoral hypoplasia has been rare. This rare defect is non‐hereditary. The affected left side exhibited deformities of coxa valgus of the hip and proximal femur, talipes valgus of the foot and aplasia of the patella; however, the tibia and fibula were anatomically normal but shorter. The right hip and lower extremity as a whole were unremarkable. Upon dissection of this 77‐year old Caucasian female died of coronary artery and peripheral vascular disease we observed unilateral hypoplasia of left femur. Vascular abnormalities were also observed. Compared to the right side the size of the left common, external and internal iliac arteries and femoral artery were smaller in diameter, and the left superior and inferior hypogastric arteries were enlarged. The discussion would include possible factors contributed to the observed defect.
