Carotid Body Denervation Attenuates Sympathetic Nerve Activity, Shifts the Pressure‐Diuresis Curve to the Left and Improves Survival in rats with Hypertensive Heart Failure

BACKGROUND Since carotid body denervation (CBD) is known to inhibit excessive sympathetic nerve activity (SNA) in animal models of hypertension and/or heart failure, we hypothesized that CBD improves survival in rats with hypertensive heart failure. METHODS We randomly allocated Dahl salt‐sensitive rats fed with high‐salt diet (8% NaCl) into 2 groups, CBD (N=20) and SHAM (N=29). At 7 weeks of age, we performed CBD. We assessed echocardiograms, 24‐hour urinary norepinephrine (uNE) excretion as a parameter of SNA and arterial pressure (AP). In some rats, we examined the relationship between urinary sodium excretion and AP with or without high‐salt diet (the pressure‐diuresis curve) at 14 weeks. Finally, we compared survival rate between 2 groups until 16 weeks. RESULTS CBD significantly reduced uNE excretion and shifted the pressure‐diuresis curve to the left at 14 weeks (figure A, B). CBD significantly preserved left ventricular ejection fraction (58.5 ± 11 vs. 67.2 ± 8.7%, p <0.05, 2D echo) and reduced biventricular weight (4.73± 0.50 vs. 4.31± 0.46 g/kg, p <0.05) as well as lung weight (5.91 ± 2.5 vs. 4.58 ± 1.0 g/kg, p <0.05) at 16 weeks. Furthermore, CBD markedly improved the survival rate with 91% relative risk reduction (figure C). Postmortem studies indicated that those rats died of heart failure.CONCLUSION CBD significantly inhibits SNA, reduces AP with leftward shifting of pressure‐diuresis curve and improves survival in Dahl salt‐sensitive rats.
