Changes in LDL particle size after aerobic training in CKD patients

Chronic kidney disease (CKD) is associated with dyslipidemia and increased risk of cardiovascular disease. We examined the effects of 48 wks of aerobic exercise training on LDL particle size in patients with CKD. Blood samples were obtained from 16 CKD patients (stage 3–4). The treatment group (TX; n = 7) performed supervised aerobic exercise 3X/week along with usual care. The control group (CG; n = 9) received usual care but did not exercise. LDL subfractionations (large = 1, medium = 2, and small = 3) were determined by polyacrylamide gel electrophoresis using the Lipoprint LDL system. Pattern A was a peak particle size ≥ 25.5 nm; pattern B: < 22.5 nm. VLDL decreased in both groups ( p = 0.04; CG: 24.4 ± 9.1 to 20.8 ± 4.8; TX: 27.4 ± 0.5 to 24.0 ± 2.3 mg/dL). LDL‐C decreased ( p = 0.02) in CG (102.2 ± 28.8 to 81.7 ± 11.1 mg/dL) but not for TX (95.6 ± 27.4 to 110.3 ± 31.5 mg/dL). LDL‐1 decreased in both groups ( p = 0.04; CG: 36.4 ± 10.0 to 29.1 ± 5.7; TX: 32.3 ± 9.7 to 30.7 ± 7.9 mg/dL). LDL‐3 increased ( p = 0.04) in TX (1.7 ± 2.1 to 3.6 ± 3.6 mg/dL) but not for CG (2.9 ± 3.7 to 1.7 ± 1.5 mg/dL). LDL‐3 increased in 4/7 patients in TX. Two subjects in the TX shifted from a pattern A to pattern B profile; no change occurred for control group subjects. We observed unfavorable changes in LDL subfractions after aerobic training in CKD patients, which warrant further investigation.
