The relationship between adherence to World Cancer Research Fund recommendations and prostate cancer aggressiveness was examined using data from the population‐based North Carolina‐ Louisiana Prostate Cancer Project. Clinical criteria were used to classify prostate cancer aggressiveness at diagnosis for 1,049 African‐American (AA) and 1,083 Caucasian‐American (CA) participants. An association between prostate cancer aggressiveness and adherence to guidelines, measured on a scale of 0–9 was assessed by examining odds ratios (OR) and 95% confidence intervals (CI) adjusted for potential confounders. A significantly inverse association was found between prostate cancer aggressiveness and adherence with each additional point associated with a 13% reduction in risk of aggressiveness. A 38% increase in likelihood of aggressiveness was associated with adherence to fewer than 4 recommendations (OR 1.38, 95%CI=1.10–1.74) as compared to those who adhered to 4 or more recommendations. Results stratified by race suggested the association is equally strong in AA as among CAs (ORs 1.36 95%CI= 1.01–1.85; 1.41 95% CI= 1.01–1.98 respectively). This study shows that recommendations aimed at preventing cancer may also be associated with the occurrence of less aggressive prostate cancer in those destined to develop the disease.
