Insulin ameliorates collagen remodeling after myocardial infarction

Aim To investigate whether insulin treatment ameliorates collagen remodeling after myocardial infarction (MI). Methods Adult male rats were subjected to the ligation of anterior descending coronary artery and randomized to receive one of the following treatments: Sham, MI+saline or MI+insulin. Myocardial collagen remodeling environment was simulated by TGF‐beta in cultured neonatal rat cardiac fibroblasts. Results Insulin significantly improved cardiac function by increased left ventricular ejection fraction after ischemia. Cardiac collagen contents were decreased post‐MI as evidenced by reduced hydroxyproline and collagen volume fraction (P<0.05). Meanwhile, immunohistochemistry staining and Western blot showed that the ratios of myocardial collagen I/III of 1 wk and 4 wk post‐MI were significantly higher than those in the sham hearts (P<0.05). Insulin treatment significantly reduced the ratios of collagen I/III compared with those in saline hearts 1 wk post‐MI (P<0.05), while there were no differences in the ratios of collagen I/III 4 wk post‐MI (P>0.05). Moreover, insulin reduced TGF‐beta induced a‐SMA expression and the phosphorylation of JNK in cardiac fibroblasts. Conclusion Insulin ameliorates cardiac collagen remodeling in post‐MI hearts via insulin‐suppressed JNK activation.
