Childhood weight and CVD risk factors in young adults: follow‐up of the 1982 Marshfield Heartwatch (HW) Cohort

Background Long term health effects of child overweight (OW) are unclear. We determined the independent effect of OW in youth on pre‐clinical CVD in young adults. Methods HW examined BMI and lipids in 3106 youth 5–15 yo. Random sampling stratified by 1982 weight (BMI < or ≥ 85 th ) was used to target subjects in 2008–09. BMI, waist hip circumference (WHC), blood pressure (BP), lipids, and ankle brachial index (ABI) were measured; historical data were obtained. Primary outcomes were abnormal ABI (<0.9) and dyslipidemia (LDL ≥ 160 mg/dL); covariates were BMI, sex, age at 1982 and 2008–09 exam. OW burden score (OWB) was calculated to reflect degree of OW since 5 yo. Results 647 subjects completed 2008–09 exams (28–32% of eligible). For males, BMI, WHC, (both p<=0.001), ABI (p = 0.001), total cholesterol (p=0.01), LDL (p=0.003) and BP (p<0.02) were higher in adults with 1982 BMI ≥ 85 th ; for females, BMI, BP and WHC (all p<0.001) were higher. BMI Z in 1982 and 2008–09 were correlated [r=0.56 (M); 0.58 (F), p<0.001]; associations between other variables were modest. 2008–09 BMI was most strongly correlated with ABI (p=0.006, M) and high LDL (p=0.002, M; p=0.046, F). OWB and 1982 BMI were not as strongly associated with ABI or LDL. Conclusion Childhood BMI did not have an independent effect on pre‐clinical CVD in young adults in our population, but was highly predictive of adult BMI.
