The distribution of cutaneous perforators (CP) form the tibioperoneal trunk, posterior tibial artery (PTA) and peroneal artery (PA), is very important for the surgical practice. We analyzed the internal diameter (ID) of the PA CP after injecting plastic mass in the arterial network of the calf followed by vascular wall dissection, on ten legs amputated from arteritic patients (80% male, aged 57–84 years). We have to highlight some particular issues to be considered: the order of arterial blockage is: (1) PTA, (2) anterior tibial artery (3) PA; the CP of the PTA are much fewer in number and suprafascial distribution territory; the CP of the PA had at the level of penetration of the fascia a significantly greater diameter in arteritic patients (mean ID 0.96 ± 0.21 D.S. mm, on an average of 3.4 CP on calf); we have shown the same issue the above lateral malleolar artery; associated path of the sural nerve and lesser saphenous vein with the median superficial sural artery (supplied by the CP of the PA), we highlighted an arterial network consisting of many interconnected parallel vessels. This study shows that the use of various types of fasciocutaneous or osterocutaneous flaps based on PA source, can be extended also to ateritic patients.
