Opposite effects on survival from Mediterranean‐like and Carbohydrate‐restricted diets in elderly Swedish men

There is an ongoing debate about dietary patterns to recommend to the public. Official dietary guidelines are challenged by advocators of Mediterranean or Carbohydrate‐restricted diets. However, long‐term head‐to‐head comparisons between diets are sparse. We therefore compared three dietary patterns with respect to 12‐year survival in a cohort of 70‐year‐old Swedish men (n=1048). Dietary misreporters were excluded. Adherence to a Mediterranean‐like diet was beneficial for survival. Compared with low‐adherent individuals, high adherent individuals to such diet exhibited hazard ratios [95% CI] of 0.45 [0.23‐0.88] for total mortality and 0.28 [0.08‐0.93] for CVD mortality (after adjustments for energy intake, life‐style and CVD risk factors). Adherence to official dietary guidelines (according to WHO criteria) was not associated with mortality, whereas adherence to a Carbohydrate‐restricted diet tended to increase mortality. Hazard ratio [95% CI] in subjects with high vs. low adherence to a Carbohydrate‐restricted diet was 1.96 [0.93‐4.13] for total mortality and 2.33 [0.80‐6.85] for CVD mortality (after adjustments for energy intake, life‐style and CVD risk factors). In summary, adherence to a Mediterranean‐like diet increased survival whereas adherence to a Carbohydrate‐restricted diet tended to increase mortality in this cohort of elderly Swedish men.
