Assessment of malnutrition in the Korean elderly using a screening tool

The DETERMINE Checklist by the Nutrition Screening Initiative (NSI) has been designed and validated to provide a single, rapid assessment of nutritional status in the elderly. This study was performed to identify the elderly at risk of malnutrition using a screening tool. This was a cross‐sectional study involving low income Korean elderly women (≥65, n=94). Socio‐demographics, lifestyle characteristics, dietary intake based on 24h‐ recall, anthropometric measures, and clinical biochemistry parameters were examined relation to nutritional status. The subjects were divided into two groups according to the sum of the DETERMINE checklist score (adequate nutritional status, NSI score <4; at risk of malnutrition, NSI score ≥ 4). Major chronic diseases of the subjects were hypertension, arthritis, and diabetes (44.1%, 33.7%, and 16.1%, respectively). Mean BMI and body fat were 25.1kg/m2 and 40.7%, respectively. However, the average intakes of energy, Vit A, Vit E, thiamin, riboflavin, Vit B 6 , folate, Vit C, niacin Ca, and Zn were below the Korean DRI (EAR). The subjects who had lower NSI score tend to have lower body weight, body fat, plasma TG, serum IL‐6, and serum CRP. Higher NSI scores were significantly correlated with lower nutrient intakes (protein, Vit A, Vit E, Vit C, thiamin, riboflavin, Vit B 6 , folate, niacin, and Ca) The NSI checklist could be used as a part of the practical nutritional screening tool of the elderly.
