Glycemic Load reflects dietary intake in hemodialysis (HD) patients

Dietary glycemic load (GL) has not been previously reported in HD patients and may be a holistic indicator reflective of the dietary profile. Study aim was to quantify GL and assess its relationship to dietary intake in a national sampling of HD patients. In a cross sectional secondary analysis, glycemic load (GL), dietary data, serum albumin, BUN, and BMI were assessed utilizing SPSS, Inc. The patients (n=155) were 46% female, 64% Caucasian, 6% Hispanic, 21% African American and 6% Asian, with a mean age of 64±14 years (mean±SD), BMI 28±7kg/m 2 , and serum albumin 3.7±0.4 mg/dL. Etiologies were: 10% type 1 diabetes mellitus (DM), 27% type 2 DM, 33% hypertension, 10% glomerular nephritis, and 59% had documented cardiovascular disease. Dietary intake analysis indicated a mean GL 102±49, kcals/kg 23±10, protein/kg 0.9±0.5, carbohydrate (CHO) g 200±87, and fat g 69±36. Glycemic load was positively correlated to total kcal, (r=.6, p<.001), kcal/kg (r=.5, p<.001), total protein (r=.4, p<.001), protein/kg (r=.4, p<.001), Cho g (r=.7, p<.001), fiber (r=.2, p<.05), fat g (r=.3, p<.001), vitamins A, (r=.3, p<.001), E, (r=.2, p<.05), C (r=.4, p<.001), riboflavin, (r=.3, p<.001), calcium, (r=.3, p<.05), iron, (r=.4,p<.001), phosphorus, (r=.3,p<.001), potassium, (r=.3,p<.001), B 12 and B 6 (r=.3, p<.05 for both), folate, (r=.2, p<.05). No correlations were observed with BUN, or serum albumin. GL positively reflects nutrient intake in HD patients.
