The purpose of this study is to investigate the relation among progesterone receptor (PR) status, apoptosis, proliferation index (PI), and histological grades in meningiomas. A total of 86 cases of meningiomas were histologically reviewed and graded (grade 1–3), and were immunostained for PR, Ki‐67, and apoptosis‐related proteins (ARPs) including Bax, Bcl‐2, Bcl‐xL, and caspase‐3. Sections were graded semi‐quantitatively for intensity and extent of immunostaining. PI was expressed as a percentage of Ki‐67 positive cells. Expression of Bax, Bcl‐2, Bcl‐xL, caspase‐3, and PR was seen in 76.4, 17.4, 84.7, 67.5, and 77.9% of cases, respectively. Bax and Bcl‐xL tended to be expressed diffusely and strongly, while Bcl‐2 was expressed weakly and focally. A mild positive correlation was observed in expression between Bcl‐xL and caspase‐3. There was no significant correlation in expression between PR and any one of ARPs, before and after accounting for gender and tumor grade. Comparing between meningiomas with PI ¡Ý2% and those with PI <2%, expression of Bax and Bcl‐xL was more significantly seen in the former group. The same trend was observed in expression of caspase‐3. Conclusion: High expression of APRs and PR was observed in meningiomas with the exception of Bcl‐2. It is unlikely that PR involves apoptosis in meningiomas. APRs appeared to be more frequently expressed in meningiomas with high PI.
