The synergistic cardioprotective effect of ischemic postconditioning and preconditioning on severe ischemia/reperfusion injury in isolated rat heart

The aim of this study was to investigate whether ischemic postconditioning (Postcon) and preconditioning (Precon) can exert accumulative effect against severe ischemia/reperfusion (I/R) induced myocardial injury. Male SD rat hearts were subjected to 30 or 60 min of global ischemia followed by 120 min of reperfusion. Postcon was fulfilled by 6 cycles of 10 s reperfusion/10 s global ischemia starting at the beginning of reperfusion and Precon was achieved by 2 cycles of 5 min global ischemia /5 min reperfusion before the ischemia. The formazan content derived from MTT was measured to reflect the cardiomyocyte viability. The level of lactate dehydrogenase (LDH) in the coronary effluent and arrhythmia scores were measured. In the isolated heart model, Postcon or Precon increased formazan content, reduced LDH release and the arrhythmia score, and improved the hemodynamic parameters in the hearts subjected to 30 min of ischemia and 120 min of reperfusion, while in the hearts subjected to 60 min of ischemia and 120 min of reperfusion, Postcon or Precond alone was also cardioprotective, but hemodynamic parameters were not ameliorated. Postcon following Precon not only reduced I/R injury, but also improved the hemodynamic parameters. The results suggest that Postcon and Precon have synergistic cardioprotective effect against severe I/R injury. (Supported by ZJSTB grant No.2005C30026)
