This paper commences with a brief historical review of addiction intervention in the New England Region of New South Wales. The concept and role of a hospital‐based assessment unit and the philosophical premises underlying its functioning are discussed briefly. The implementation of a pilot service at the Armidale and New England Hospital is outlined and some of the dilemmas encountered in consolidating the service are highlighted. Some future issues are examined.
