1. The relationship between the duration of diabetes and the onset of diabetic glomerulosclerosis cannot be stated with any certainty, chiefly because the time of onset of diabetes is so obscure. 2. It is not possible to state at this time whether there is any relationship between the degree of control of diabetes, or the type of control, and the onset of the renal complication. . . . The question of control is obviously related to the duration of diabetes because the longer the latter the more tedious and difficult the former. 3. Duration of life after onset of diabetic glomerulosclerosis: This information is difficult to obtain because of the uncertainty as to the time of onset of the renal disease. 4. From our experience, the study of the urinary sediment when properly related to other clinical data can definitely establish the diagnosis of diabetic glomerulosclerosis. . . . Careful examination of urinary sediments for anisotropic fatty cells becomes an important, yet simple, diagnostic aid in diabetic glomerulosclerosis. 5. From all available data in the literature and from our own experience, the prognosis of diabetic glomerulosclerosis is hopeless at present. Therefore, we can offer no specific therapy. It is only possible to render symptomatic and supportive care. . . . 6. Diabetes has probably progressed to half its course by the time it is clinically recognized. The opinion has been generally held that accelerated vascular damage is the inevitable fate of the diabetic, regardless of the severity of the diabetes or its treatment.
