Pancreas allograft transplantation (PTX) when successful is an effective modality for the control of glucose metabolism in diabetic patients [1, 2]. Side‐effects are mainly related to surgical complications, immunosuppressive therapy and graft rejection. Hypoglycaemia is an unexpected complication that has been reported [3, 4]. We report two patients having recurrent hypoglycaemic events following pancreatic transplantation and review the literature.
