The case of a 32‐year‐old woman with neuroleptic malignant syndrome and severe haemorrhagic thrombocytopenia is presented. Detailed investigations were negative, and the thrombocytopenia resolved along with her fever, obtundation, autonomic instability and oculogyric crisis. No prior similar cases have been described in the literature. A discussion of neuroleptic malignant syndrome and neuroleptic‐related thrombocytopenia follows, in addition to the possible mechanisms of the thrombocytopenia in this case.
