Summary We report a patient with granulomatous mycosis fungoides whose disease transformed into a high grade blast lymphoma with egocentric features within 12 months of the initial diagnosis. This repudiates previous claims that granulomatous inflammation is protective in cutaneous T‐cell lymphoma.
