MRI/TRUS fusion is an added value in the detection of prostate adenocarcinoma as it allows targeted prostate biopsy of suspicious areas.Clinical case65 years old male, followed by elevated PSA. Submitted to 2 conventional transrectal biopsies, both negative for prostate cancer . Due to persistent elevation of the tumor marker it was performed a biopsy with cognitive image fusion, with a second negative result. Finally, he performed a MRI/TRUS fusion biopsy and a Gleason 8 (4+4) PC was diagnosed.DiscussionIn a near future, the fusion of MR images with real time transrectal ultrasound for guidance of suspicious lesions will result in more accurate biopsies and a reduction on the number of procedures for the detection of clinically significant disease. More research is needed to evaluate the role of this platform in cancer detection, focal treatment and active surveillance, as to establish wich patients may benefit from it
