Highlights• CSF enhancement on post-contrast FLAIR images is a novel marker for BBB leakage. • This neuroradiological marker is frequently observed in neurological diseases. • Post-contrast FLAIR CSF enhancement is associated with higher age and brain atrophy. • There is large methodological heterogeneity between studies that use this technique. • We provide recommendations for future methodological standardization.
