centered pattern of high molecular weight neurofilament (NF-H) pathology in brains with SVD. Using a digital pathology approach we tested the hypothesis that peri-vascular NF-H pathology is associated with SVD.Methods: Subcortical white matter was examined in older people with severe AD pathology and minimal SVD (n1⁄411), SVD neuropathology and absent/minimal AD pathology (Braak stage< IV; n1⁄416), aged controls (n1⁄412) and in young control subjects (n1⁄412). In digital scanned sections we quantified i) extent of NF-H labelling (% area fraction) using a densitometry algorithm, ii) fraction of small arteries with vessel-centered NF-H, iii) degree of axonal bulbing (categorical score, 0-7). Results: All cases including young controls showed some degree of NF-H-based axonal pathology in subcortical white matter. Extent of NF-H labelling was greatest in brains with severe AD pathology (significantly more than aged controls, p1⁄40.010; 1-way ANOVA). Fraction of small arteries with vessel centered NF-H was greatest in brains with SVD and minimal AD (significantly more than AD brains, p1⁄40.028). The degree of axonal bulbing was significantly associated with fraction of small arteries with vessel-centered NF-H (r1⁄40.850, p1⁄40.0021). Conclusions: Peri-arterial NF-H-based axonal pathology may be a feature of SVD, hence of VCID.
