consensus-based case conference attended by the entire interprofessional team. At a feedback visit, PWD and care partners are given a diagnosis and tailored treatment plan that often includes in-house counseling, medication management, and cognitive-communication therapy with the ultimate goal of transitioning care back to their primary care physicians. Counseling services include connecting with and navigating local resources, as well as psychotherapy, education, emotional support, and family counseling. Value-based outcomes assessed include time from initial diagnostic visit to feedback, satisfaction with care, use of cognitive-communication strategies, connections to community resources, and reduction of distress and burden in care partners.
