Background: In this study, we compared HDS-R and other common neuropsychological tests used in community.Methods:602 community derived participants were enrolled in Tajiri, Miyagi prefecture, Japan. 407 of them are normal (CDR1⁄40), 159 areMCI (CDR1⁄40.5), 36 are dementia (CDR above 1). HDS-R, MMSE (mini-mental status examination) and CASI (cognitive abilities screening instrument) were performed in each of our participants. Results: HDS-R (AUCHDS-R:0.80) and CASI (AUCCASI:0.81) showed higher screening accuracy than MMSE (AUCMMSE:0.76) on separating cognitive impaired subjects (CDR above 0.5) in community. The Spearman’s correlation coefficient of HDS-R, CASI, MMSE with CDR-SOB (clinical dementia rating scale, sum of boxes) are -0.59, -0.65, -0.54 separately. Conclusions: HDS-R emphasizes more on executive and language function. Which makes it a better screening tool in community. Besides, it correlates with subject’s clinical dementia condition more.
