Contact e-mail: Rashid_Deane@URMC. Rochester.edu Background:There are many mechanisms for the clearance of molecules from brain. These include but not limited to receptor-mediated transport across the blood-brain barrier (BBB) and cellular uptake into cell, ISF diffusion/bulk flow and degradation. In the case of amyloid-b (Ab) peptides, which are relatively larger macromolecules, receptor-mediated transport across the BBB and cellular uptake into other brain celltypes appears to be the main mechanism followed by ISF diffusion/bulk flow and degradation. It is unclear on how these processes are affected by sleep/wake cycle, and other brain states. Clearance from the interstitial fluid (ISF), i.e., the parenchyma, is affected by ISF and cerebrospinal fluid (CSF) flow rates, in addition to changes in density of cell membrane-bound receptors. In the case of Ab, these receptors, such as low density lipoprotein receptor-related protein-1 (LRP-1) and receptor for advanced glycation end products (RAGE) are altered in the aging and Alzheimer’s disease (AD) brains.
