success of REACH in Tarrant County helped secure funding from the Rosalynn Carter Institute for Caregiving and federal grant sources through local partnerships to expand the program to rural counties. Conclusions: Embedding evidence-based programs in a local agency is an effective way of translating such programs into the community setting especially when the EBP aligns with the agency’s existing mission. The Association’s infrastructure naturally supports REACH which deepens the chapter’s offerings to its constituents. Such a combination appeals to a broader funding pool and widens partnerships for sustainability. Embedding of EBP in a local agency can achieve sustained positive outcomes that are cost effective.
