P3‐248: STRATIFICATION OF INDIVIDUALS FOR PET AMYLOID POSITIVITY AND ALZHEIMER'S DISEASE RISK USING POLYGENIC RISK SCORE ANALYSIS: NEW OPPORTUNITIES FOR CLINICAL TRIAL DESIGN

analyzed and compared plasma glycated hemoglobin (HbA1c) and fasting blood glucose levels in patients with Alzheimer’s disease and in a control group. Their associations with the development of AD were also investigated. Methods:Measures of fasting plasma glucose levels were studied in 60 patients with AD, (Mean age 71.566.9 years, range 59-85 years, 30 women), (DSM-IV; NINCDS-ADRDA criteria); recruited in a community elderly in Skikda-Algeria (2015-2017); and in 60 elderly controls, (Mean age 71.367.3 years, range 58-85 years, 30 women). Global cognition was estimated using the mini mental status examination score and dementia severity assessed by the clinical dementia rating (CDR) global score scale. Results: Patients with AD had lower mean (6 SD) fasting glucose level 88.6 6 6.3 mg/dL (Range 73-103), than control 93.0 6 7.3 mg/dL (range 75-106), p1⁄4 0.0006. Plasma glycated hemoglobin level was significantly lower in cases group: 5.24 6 0.34% (Range 4.45.9) than in control group: 5.49 6 0.29% (Range 4.8-6.1), p< 0.0001. The odds ratio [95 percent confidence interval] of AD for the highest tertile of HbA1c levels (Range: 5.6-6.1) compared to the lowest tertile (Range: 4.4-5.2) was 0.2070 [0.0798– 0.5367], p1⁄4 0.0014. The odds ratio [95 percent confidence interval] of AD for the highest tertile of fasting blood glucose levels (Range: 95-106) compared to the lowest tertile (Range: 74-85) was 0.2032 [0.0797–0.5178], p1⁄4 0.0008. Plasma HbA1c and glucose levels did not correlate significantly with dementia severity or cognitive score in AD group. Conclusions:The results show significantly lower plasma glucose and glycated hemoglobin levels (both within the normal range) in patients with Alzheimer’s disease compared to control. Subjects with ”high normal” fasting plasma glucose level and “high normal” HbA1c level, have a lower risk for developing Alzheimer’s disease compared to those with “low normal” level. Such findings stress the need to unravel the relationship between Alzheimer’s disease and peripheral glucose metabolism.
