dementia-capable organizations; training providers and community based organizations; and delivering multidisciplinary services to enrolled clients. A two-pronged model was used: short-term case management and development of Care Circle support. Casemanagement included multiple home visits to assess client needs and preferences, connection to community services, and regular follow-up. Care Circle development comprised of identifying individuals, organizations or community services who could provide on-going, longterm support. It also included disease education and care planning consultation. Expected outcomes were: enhanced access to community resources, reduced isolation, improved quality of life and increased awareness and dementia capacity of community organizations. Outcomes were measured by QOL-AD, CANE, satisfaction survey, and ACL. This poster presents preliminary data on expected outcome and uses case studies to outline the use of the Live Alone Algorithm.
